Dental ERx Telehealth Consent & Payment Policy

By submitting this consultation and completing payment, you consent to receive teledentistry services through Dental ERx.

A licensed dental provider will review the medical history, symptoms, and any photos or information you submit for the purpose of a remote dental consultation.

You understand that teledentistry has limitations because the provider cannot perform a full in-person dental exam or obtain x-rays. Recommendations are based only on the information you provide, and a definitive diagnosis and treatment require an in-person dental visit.

Any medications prescribed are intended to provide temporary relief until you are seen by a dentist in person. Prescriptions are issued only if clinically appropriate, and Dental ERx does not prescribe controlled substances or narcotic pain medications.

If you experience severe swelling, difficulty breathing or swallowing, fever, spreading infection, uncontrolled bleeding, or other medical emergencies, you should seek immediate care at the nearest emergency room.

The consultation fee covers the provider’s professional review of your case and is non-refundable once your consultation has been submitted for review, regardless of whether medication is prescribed.
