Most people have been there a dull ache turns sharp at 11 PM on a Sunday, or a crown pops off the morning before a big meeting. The question that follows is almost universal: Do I really need to go in, or can this wait?
The answer depends on what’s actually happening in your mouth. Some situations give you a window. Others don’t. Knowing the difference can protect your health, your tooth, and your wallet.

What Makes a Dental Problem an Emergency?
A dental emergency isn’t just pain it’s any situation where waiting could lead to permanent damage, spreading infection, or tooth loss. At Dental ERx, we see patients who waited too long and patients who came in right away. The outcomes look very different.
Here are five situations where you should not wait for a regular appointment.
1. Severe, Unrelenting Tooth Pain
Pain that comes and goes when you eat something cold is usually a sensitivity issue. Pain that is constant, throbbing, and getting worse is something else.
That escalating ache is often a sign of pulpitis inflammation inside the tooth or the early stages of a dental abscess. Left untreated, the infection can spread to surrounding teeth, your jaw, and in serious cases, your airway.
Don’t mask it with ibuprofen and go to sleep. Come in.
2. A Dental Abscess
An abscess is a pocket of infection. Signs include:
- A pimple-like bump on your gum
- Swelling in your jaw, cheek, or under your chin
- Fever alongside tooth pain
- A foul taste or discharge in your mouth
This is a true emergency. Dental infections do not resolve on their own, and untreated abscesses can become life-threatening. If you have facial swelling with difficulty breathing or swallowing, go to the nearest emergency room immediately.
3. A Knocked-Out or Partially Dislodged Tooth
A knocked-out permanent tooth has roughly a one-hour window for successful reimplantation. Every minute outside the socket matters.
If you lose a tooth:
- Handle it by the crown (the white part), not the root
- Rinse gently don’t scrub
- Place it back in the socket if possible, or keep it in a container of milk or saliva
- Get to a dentist immediately
Partially dislodged teeth loose, pushed back, or tilted are equally urgent. Call us or use our 24/7 teledentistry line for immediate guidance while you’re on the way.
4. A Broken Tooth With Exposed Nerve
Chips and minor cracks often cause sensitivity but not true emergencies. A break that exposes the inner layer of the tooth called the dentin or pulp is different.
You’ll usually know because the pain is sharp, immediate, and worsens with air, temperature, or pressure. Left exposed, the nerve is vulnerable to bacterial invasion. Same-day care can often save the tooth.
5. Lost Filling or Crown With Pain or Sensitivity
Crowns and fillings fall off. It happens. If there’s no pain and the tooth feels stable, you likely have a short window but don’t ignore it. The exposed tooth will wear quickly and can decay fast.
If the tooth underneath is sensitive to pressure, temperature, or air don’t wait. That sensitivity signals the tooth structure may be compromised.
